Creating your personal profile

The CARP Health 360 system starts with a personal profile you create yourself. It includes such items as emergency contacts, physician names and contact information, conditions you may have (illnesses, allergies, etc.) and medications, and other “base line” data. This guided process is convenient, quick, and easy

We can help walk you through the process

If you prefer, we can fill out your personal health profile together. When you make your initial appointment, our health concierge will walk through the forms and enter all the information you provide. You can also collect more detail from your doctor before the call to ensure your profile is as complete as possible.

Your data is stored online in our secure system. We use double encryption, and follow the strictest ISO 27001 standards to ensure total security. We offer two convenient ways to access that data.

The Health 360 Digital Health Portal

Once you’ve enrolled, you receive a wallet-sized card (you can also print it out yourself the moment you’ve enrolled) that shows a website address and PIN number. In case of an emergency, simply give this card to a first responder, and they can to to the website, enter the PIN number, and have immediate digital access to your information.
