Survey: Mississauga Halton LHIN Would Like to Hear from You

The Minister of Health and Long-Term Care has appointed a Home and Community Care Review Expert Group (Expert Group) to develop recommendations for improving the home and community care sector. Specifically, the Expert Group is looking at:

  1. Opportunities to make service delivery more consistent across the Province in order to promote equal access to quality home and community care and services across all communities;
  2. Opportunities to make provincial funding for services and investments in home and community care services more consistent across the Province in order to improve transparency, and maximize the funding available for patient/client care and services;
  3. Opportunities for innovations and new approaches to care in order to improve the patient/client experience, quality, choice and value for money.

To support the work of the Expert Group, the Mississauga Halton Local Health Integration Network (LHIN) is asking patients, family members and/or primary caregivers who have received and/or used home and community care services during the past year to complete a short online questionnaire. The information obtained from the questionnaire will be used by the Expert Group to inform its recommendations to the Minister.

The health needs of local communities are best understood by those who live and work in them, which is why community input and feedback is invaluable when shaping the local health care system. Share your ideas on how to improve home and community care.
