But there are always exceptions to every rule. For instance, I recently asked an elderly patient, “Is there anything else you want to discuss?” She replied, “I’m embarrassed to tell you that my leg muscles are now so weak I can’t get up off the toilet seat.”

To me, this is an emergency, with institutional care looming as a real possibility. I’d risk snake oil or any of the potential complications of creatine and CLA in order to avoid being forced to leave my own home. The fear of institutional care in my last years would prompt me to pull out all the stops to avoid it. •
