Empowerment for Self-Management & Independence in Face of Parkinson’s Disease
The Partnership: CARP and Yee Hong Centre for Geriatric Care
Public education and caregiver support is the focus of a new partnership between CARP and Yee Hong Centre for Geriatric Care. A series of chronic disease self-management workshops will be provided to CARP members and members of the broader community, led by certified Chronic Disease Self-Management Program (CDSMP) leaders at Yee Hong. Yee Hong is the “gold standard” in providing culturally appropriate long-term care and community services, and is responding to the growing need for public education in chronic disease management. The series starting on October 5, 2011, is a 10-session Parkinson’s Disease Management Program.
Who Should Attend
Parkinson’s disease sufferers and their caregivers will benefit from this program. Caregivers of a registered participant attend FREE.
Benefits for Attending
Attendees will acquire hands-on strategies and practical tips from an interdisciplinary team of health care professionals, including Certified CDMSP Leader, Registered Pharmacist, Occupational Therapist, Physiotherapist and Social Worker. The workshops are based on the model patented by Stanford University to empower seniors and pre-seniors with the knowledge and techniques to manage chronic diseases and maintain an active and independent lifestyle.
Hands-On, Excellent Quality Experience
The weekly sessions are 2.5 hours each. All 10 sessions are interactive. The program is limited to 12 participants. The small-group setting gives participants plenty of direct instruction and practice opportunities. Topics covered include education about the disease, medication management, mobility improvement through exercises, life skills and training for activities of daily living.
