Sept. 18, 2012 – CARP met with Dr. Sinha, the Expert Lead for Ontario’s Seniors Strategy. We presented to him CARP’s care continuum “One Patient” model and we will be sending a copy of our “One Patient” model to all health ministers in the near future.
