Mississauga, ON – According to Statistics Canada, approximately 3.6 million Canadians do not have a family doctor. If you’re one of the many without a healthcare provider, Health Care Connect can help.

Health Care Connect is a free service designed to help Ontarians find a family healthcare provider such as a doctor or nurse practitioner who is accepting new patients in your community.

To be eligible for this service, you must not be enrolled with a family healthcare provider, have a valid Ontario Health Card, and an up-to-date mailing address. If you are currently enrolled with a family doctor but wish to end your enrolment, you can speak to him/her directly or call 1(888)218-9929.

Registering with Health Care Connect is simple; call the number below or go online. The Mississauga Halton CCAC has three Care Connectors (registered nurses) working with primary care physicians to find the most suitable match for you. After answering a few questions, your name will be added to the list, and notified if any healthcare providers in your community are accepting new patients.

For more information call 1(800)445-1822 or visit www.ontario.ca/healthcareconnect.

The Mississauga Halton CCAC coordinates in-home and community support services, information and referral, and long-term care placement. It is funded by the Ministry of Health and Long-Term Care and serves a population of more than one-million residents in the communities of South Etobicoke, Mississauga, Oakville, Haltom Hills and Milton.

The Mississauga Halton CCAC is funded through the Mississauga Halton Local Health Integration Network (MH LHIN).
